Accent Home Care is committed to supporting older Australians through the Commonwealth Home Support Programme (CHSP), delivering services such as personal care, domestic assistance, community nursing, and social support.
We are currently seeking a dedicated and organised CHSP Coordinator to join our team and play a key role in ensuring that the care needs of our consumers are met through regular plan reviews and personalised support.
About the RoleAs the CHSP Coordinator, you will be responsible for:
Conducting annual care plan reviews to ensure services align with each consumer's evolving needs
Undertaking face-to-face and phone-based consumer meetings, including home visits
Documenting all care plan outcomes and updates accurately in electronic records
Ensuring a high level of customer service, empathy, and professionalism in all consumer interactions
Working both on the road and from our Bayswater office
The ideal candidate will have:
✔️ Experience in community, aged care or client coordination roles✔️ Strong communication and interpersonal skills - confident in both face-to-face and phone interactions✔️ Ability to assess consumer needs and match them with appropriate CHSP services✔️ High attention to detail with strong documentation and reporting skills✔️ Ability to manage time and schedules effectively across home visits and office work✔️ A current driver's license and access to a reliable vehicle
