Essential Requirements:
For nursing staff - Current unrestricted Authority to Practice as a Registered Nurse with the Nursing and Midwifery Board of Australia. Relevant post-registration qualifications in mental health and at least 3 years experience working in mental health or a combination of equivalent experience and qualifications For allied health professionals - Occupational Therapist: Bachelors or postgraduate degree in occupational therapy and hold general registration with Occupational Therapy Board of Australia under Australian Health Practitioner Regulation Agency (Ahpra). To be appointed as a Level 3 Senior Clinician, evidence must be provided of minimum three years clinical experience as an Occupational Therapist and demonstrated experience of extensive specialist knowledge or a high level of broad generalist knowledge within the discipline as defined by the award definitions of a Level 3 Health Professional. Social Worker: Bachelors or postgraduate degree in social work and hold professional membership or be eligible for full membership with the Australian Association of Social Workers (AASW). To be appointed as a Level 3 Senior Clinician, evidence must be provided of minimum three years clinical experience as a Social Worker and demonstrated experience of extensive specialist knowledge or a high level of broad generalist knowledge within the discipline as defined by the award definitions of a Level 3 Health Professional. Psychologist: Minimum of a four-year accredited psychology degree and hold general registration with Psychology Board of Australia under Australian Health Practitioner Regulation Agency (Ahpra). Approved or eligible for approval as a Psychology Board of Australia Supervisor. Please Note: This position is funded as a Clinical Nurse Specialist Level 2, Health Professional Level 3, and Psychologist level. If you have the required experience and qualifications to meet the award requirements, you are eligible to apply for the position at the advertised grading. If you do not yet meet the award requirements, you are still eligible to apply for this position as a Registered Nurse 2-8 or Health Professional Level 2, but with a commitment to develop the required skills, knowledge, and experience, of the more senior position, within 3 years of being appointed to the position. Please indicate which position you are applying for in the selection criteria.- The CTT is a statewide service, supporting people from several correctional centres across Sydney metropolitan and regional NSW, and liaising with all NSW Health Local Health Districts. There is a requirement to travel to various locations across NSW on occasion.

The Role
The Community Transitions Clinician will support people who experience severe mental illness to access the necessary care and support required to ensure a successful transition from custody to the community. The Community Transitions Clinician will be part of a multidisciplinary team that works in partnership with the Custodial Mental Health Service, primary health networks, Community Mental Health Services and other government and non-government organisations (NGOs) to provide a collaborative approach to ensuring continuity of care. By understanding patients clinical, psychosocial and behavioural vulnerabilities, and social and emotional wellbeing needs, the Community Transitions Clinician will utilise general case management and discipline specific skills to link patients to relevant services in the community and support their initial reintegration to community care. The Community Transitions Team is one of the three teams that comprise the NSW Community Forensic Mental Health Services. The team is community based; but provides assertive prison in-reach services to support peoples transitions back into the community. Its aim is to develop mental health care plans, establish necessary therapeutic supports for patients, and provide follow-up community care to patients leaving custody. The team is recovery-oriented and promotes positive health outcomes and community participation
