About the role

Are you passionate about empowering people with disability to live their best lives? Do you have a knack for navigating the NDIS and building strong, trusting relationships? We're looking for a dynamic and compassionate individual to join our team as a Support Coordinator & Recovery Coach at Including You.  As a Support Coordinator / Psychosocial Recovery Coach, you will play a pivotal role in helping individuals enhance their overall well-being, develop coping strategies, and achieve their personal goals.

As a Support Coordinator and Recovery Coach, you'll work closely with NDIS participants to:

  • Support Coordination: Help participants understand and implement their NDIS plans, connect with services, and build their capacity to manage supports.

  • Recovery Coaching: Use a person-centred, strengths-based approach to support individuals with psychosocial disability in achieving their recovery goals.

  • Collaborate with providers, clinical services, and informal supports to ensure seamless service delivery.

  • Maintain accurate case notes, reports, and documentation in line with NDIS and organisational requirements.

·       Promote choice and control in all aspects of service delivery.

Reporting to the Regional Manager - you will:

  • Work with NDIS participants, families, and other stakeholders to implement, monitor and support the review of NDIS plans

  • Achieve targets as identified by the company and the Regional Manager, in line with the strategic business plan and guidelines as set out by the NDIS

  • Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports

  • Connecting participants with both formal and informal supports to improve their quality of life

  • Ethical, customer-focused practice to provide support to the participants to achieve their goals

  • Identify and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements

  • Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved, and services are delivered in accordance with the participant's plan.

What we are looking for

·        Knowledge of the NDIS and experience in Support Coordination and/or Recovery Coaching

·       A recovery-oriented and trauma-informed practice framework

·       Strong communication and advocacy skills

·       Ability to work independently, manage time effectively, and juggle competing priorities

·       Qualifications in Community Services, Mental Health, Social Work, or a related field (desirable)

·       Current Driver's Licence and reliable transport

·       Current NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)

  •  A solid understanding of quoting, budgeting, and financial procedures with strong financial acumen.

  • Work autonomously and as part of a small team, taking responsibility for assigned tasks and actively contributing to team meetings and planning sessions

 The Team

This is hybrid role which is 2 days in the office and 3 days working from home (first 3 months, 5 days per week in the Bundoora office)

An NDIS Workers Screening Check and Working with Children Check are mandatory requirements of this role. If you do not already hold these, you will be required to obtain them prior to employment.

Qualifications

Cert IV, Diploma or Degree in Community Services, Disability, Mental Health, Individual Support or similar preferred
