About The Position

Location: Port Augusta 

Status: Part Time FTE 0.8

The Community Mental Health Program provides psychological support including goal setting, transition planning, and assistance to transition to NDIS services. This is achieved through flexible and intensive case management to support individuals to reduce levels of distress and improve their wellbeing.

As a Community Mental Health Case Manager, you will provide personalised, recovery-oriented support to people living in the community, with a focus on Aboriginal and Torres Strait Islander people. You will work collaboratively with clients to achieve their recovery goals through one-to-one support, shared services, workshops, and group facilitation.

You will:

  • Work with clients to develop independent living skills and a meaningful life.
  • Purposefully engage using intervention, motivational, trauma-informed, and conflict resolution techniques.
  • Provide support to families and carers by facilitating meetings where relevant.
  • Support clients to source and retain safe, affordable, and sustainable housing.
  • Undertake group work and collaborate with local service providers and clinical partners.
  • Maintain accurate case notes and reports within required timeframes.

It is desirable for you to have:

  • Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
  • Strong relationship building and communication skills.
  • Awareness and understanding of the NDIS.

It is essential for you to have: 

  • Current Australian Drivers Licence.
  • Current NDIS Worker Check.
  • Relevant experience, in a paid or voluntary capacity.
  • A minimum Certificate IV qualification in a Human Services related discipline (e.g. Mental Health, Peer Work) or a willingness to work towards a qualification.

A tertiary qualification will be highly regarded.

You may also need to be available for some out of hours work and travel between Uniting Country SA sites of interstate as required.
