Clinical Care Coordinator at APS 6 Level

Join a passionate and high-performing mental health team supporting Australia's communities. As an Clinical Care Coordinator, you'll deliver trauma-informed, client-centred care to those who have mattered most in our country - and their families - when they need it most.
What You'll Be Doing
Deliver high-quality, evidence-informed clinical services including intake, risk assessment, brief intervention, care coordination, and referrals.
Manage a caseload of clients with complex or multiple needs using a secure Client Management System.
Liaise with DVA, ADF, ex-service organisations and other key stakeholders to coordinate support and enhance service pathways.
Promote recovery-based care within a stepped-care model.
Participate in service planning, program delivery, evaluation, and quality assurance activities.
Co-facilitate group treatment programs and community outreach initiatives.
Attend clinical supervision and CPD activities to maintain your registration and support best practice.

✅ Mandatory Requirements
To be eligible, you must:
Hold (or be eligible to obtain) a Working with Vulnerable People or Working with Children check
PLUS one of the following:
AHPRA-registered Psychologist, Occupational Therapist or Registered Nurse
Social Worker with AASW membership
Masters-qualified Counsellor registered with PACFA (Clinical) or ACA (Level 3 or 4)
