In this dynamic, field-based role, you'll manage care services for clients across the Northern Rivers — building strong relationships, tailoring care plans, and ensuring that every client feels seen, heard, and supported.
You'll work closely with our team to deliver a seamless client experience while helping us grow our impact across the region.

Your Day-to-Day Might Include:
Meeting with new and existing clients to assess needs and develop personalised care plans
Working closely with families to create support strategies that feel right
Reviewing care goals regularly and adjusting plans as needed
Coordinating with our support team to ensure smooth delivery of services
Documenting client interactions, updating records, and sharing relevant info with the team
Supporting our caregivers with feedback, updates, and encouragement
You'll Be Great in This Role If You Have:
Experience in aged care, case management, or community services
A minimum of 1-2 years of experience in aged care, case management, or community services
Certificate III in Aged Care or similar (desirable, but not essential)
Experience with Home Care Packages (HCP) - highly regarded
Strong admin and communication skills
Confidence using Microsoft Office (Excel, Outlook, Word)
Advanced proficiency level in Microsoft Office applications including Excel, Outlook, and Word
A warm, approachable style and a genuine passion for helping others
A current Australian Driver's Licence and your own vehicle
Flexibility to travel throughout the Northern Rivers region
