Primary objective As a member of the multidisciplinary health team, provide Aboriginal and Torres Strait Islander Primary Health Care services in a culturally safe and effective manner; through prevention, early detection, early intervention; and treatment of illness; and within a defined scope of practice. Context statement The Aboriginal Health Practitioner classification level AHP2 is an experienced practitioner. It requires the occupant to operate with a level of independence against a specific set of rules and procedures set by the health service provider and may be required to assist with responding to medical and community emergencies. This level is also considered appropriate for experienced practitioners re- entering the clinical workforce and requiring upskilling and recently of practice. This role may be required to provide second on-call (after hours) emergency services on a rotational basis. Key duties and responsibilities 1. Provide Aboriginal and Torres Strait Islander primary health care practice to individuals and their families within a defined scope of practice and health care plans; and support them to navigate the health system, ensuring continuity of care across the healthcare continuum. 2. In partnership with other health professionals provide health education and promotion programs in the community, including engaging local community members to strengthen community involvement in health service delivery. 3. Provide mentoring and support to Aboriginal Health Practitioner Trainees including local workplace induction. 4. Actively participate and complete all workplace essential training requirements as identified by the organisation and workplace. 5. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken in order to ensure high quality, safe services and workplaces. Selection criteria Essential 1. Current registration with Ahpra as an Aboriginal and/or Torres Strait Islander Health Practitioner. 2. Minimum AQF level 4 as per NTPS Aboriginal Health Practitioner 2022-2025 Enterprise Agreement and/or learning agreement demonstrating a commitment to achieve this. 3. Proven minimum 2 years experience employed as a registered Aboriginal and/or Torres Strait Islander Health Practitioner in a health care environment with experience and knowledge of health information systems used; in particular, patient care systems; and data collection. 4. Demonstrated local Aboriginal cultural knowledge, respect, and safety at the individual and families levels; maintaining appropriate confidentiality; and ensuring clients understand information received and are providing informed consent. 5. Ability to self-manage within a multi-disciplinary health team and provide primary health care services against national health care standards in a culturally safe and respectful manner. 6. Must be able to provide confirmation of Aboriginality; obtain a criminal history check and clearance; and working with childrens clearance (Ochre card). Desirable 1. Current NT drivers licence, or the willingness and or ability to obtain.
