About the Role
As a Care Coordinator (Level 5), you will oversee a caseload of approximately 20 participants, manage staff coordination, and ensure all services meet NDIS quality and compliance standards. This role includes preparing for internal and external audits, monitoring KPI outcomes, and supporting staff in complex service delivery.
Key Responsibilities:
Coordinate participant supports and implements care plans across SIL and Community Access programs.
Maintain case notes, reports, incident records, and NDIS documentation.
Prepare for internal and external NDIS audits and ensure compliance with Practice Standards.
Manage rosters, shifts, and appointments efficiently.
Support staff in managing participants with behaviours of concern, restrictive practices, and high-intensity needs.
Liaise with families, stakeholders, and clinicians to ensure collaborative support delivery.
Track KPIs and service outcomes, reporting to management as required.
Contribute to quality improvement and continuous service development.
About You
Bachelor's degree in social work, Psychology, Community Services, or related discipline.
Experience supporting participants with complex needs and challenging behaviours.
Strong knowledge of NDIS Practice Standards, restrictive practices, PBS, compliance, and audit processes.
Ability to manage multiple participants, staff, and operational tasks.
Excellent organisational, problem-solving, and communication skills.
Professional, empathetic, and confident in managing both participant and staff needs.
