About the role 

As an Onsite Pharmacist at Aspect Health, you will work directly at our Aged Care customers' facilities, collaborating closely with healthcare teams—including doctors, nurses, and allied health professionals—to provide tailored pharmaceutical care. Your role will be crucial in ensuring the safe and effective use of medications, conducting regular medication reviews, and improving the quality of life for aged care residents. 

Key Responsibilities 

• Work Onsite at Aspect Health Aged Care Customers' facilities to provide direct support and expertise within the aged care setting. 
• Work closely with doctors, nurses, and allied health professionals to deliver tailored pharmaceutical care. 
• Review medication management policies and administration procedures to maintain high standards of medication management and safety. 
• Support efficient and effective medication supply from the Pharmacy to improve medication supply processes to meet residents' needs. 
• Liaise with residents and their families to provide clear communication and education about medication use, safety, and potential side effects. 
• Ensure Compliance with Aged Care Quality Standards and Relevant Legislation and adhere to regulatory requirements and industry standards. 
• Promote the Safe and Effective Use of Medicines and implement strategies to reduce adverse drug reactions and enhance medication safety. 
• Enhance residents' well-being through expert medication management and personalised care. 

About You 

To succeed in this role, you will: 

• Be a registered pharmacist with AHPRA. 
• Have experience in aged care, community pharmacy, or hospital pharmacy. 
• Completion of training as needed for Aged Care Onsite Pharmacist. 
• Demonstrate a strong understanding of medication-related needs of elderly residents. 
• Possess excellent communication and interpersonal skills, with a collaborative approach to working with healthcare teams. 
• Be committed to delivering high-quality, person-centred care to improve resident outcomes.
