Join Uniting’s Commonwealth Psychosocial Support (CPS) Program and help people living with severe mental health challenges to achieve their recovery goals and feel more connected to their community.
This is a community-based role, where you’ll work alongside consumers to provide practical, recovery-focused support that helps them navigate everyday challenges, strengthen their independence and build the skills and confidence they need to thrive.
You'll be part of a supportive multidisciplinary team, working alongside Mental Health Support Worker, Occupational Therapist and Senior Mental Health Support Worker.
About the Program
The Commonwealth Psychosocial Support Program (CPS) supports individuals experiencing severe mental health challenges to access short-term, recovery-focused care that enhances wellbeing and daily functioning.
Our specialist team works collaboratively with consumers to develop tailored support plans and deliver meaningful interventions through individual and group-based support.
About you in the role
You are compassionate, resilient and passionate about supporting people on their recovery journey. You enjoy meaningful community work and thrive in a collaborative team environment.
Deliver recovery-focused outreach support within the community
Assist consumers with navigating supports including NDIS, Centrelink and housing services
Support individuals to build independence, resilience and daily living skills
Develop and review individual support plans in partnership with consumers, carers and clinicians
Promote improved physical and mental wellbeing through capacity-building activities
Maintain accurate documentation and manage community-based outreach effectively
Work within a supportive multidisciplinary team with mentoring from a Senior Mental Health Support Worker

Essential Criteria
Certificate IV in Mental Health or Community Services (minimum)
Current unrestricted driver’s licence and access to own vehicle
Understanding of or willingness to learn recovery-oriented practice
Strong communication and relationship-building skills
Ability to identify and respond appropriately to risks
Comfortable balancing outreach work with administrative responsibilities
Demonstrated ability to support consumers with individual care planning
