Caring Approach is a dynamic, vibrant, supportive, and caring organization. We provide tailored in-home support and community care for the elderly, people with both physical and intellectual disabilities, and people needing short-term assistance after being discharged from the hospital. We are seeking caring and professional individuals to become part of the Caring Approach team to promote independence for people living at home. General duties include: Assess participant needs, goals, abilities and support requirements in line with their NDIS plans. Assist in developing and implementing individualised support plans and community access programs. Support participants with social, emotional, intellectual and physical disabilities to build independence and life skills. Monitor participant progress and recommend adjustments to support strategies where required. Liaise with families, guardians, support coordinators, allied health professionals and other service providers. Provide guidance to disability support workers in implementing participant support plans. Promote participant choice, control, dignity, privacy and community inclusion. Identify and report risks, incidents, changes in participant needs and safeguarding concerns. Maintain accurate case notes, progress reports and service delivery records. Ensure services are delivered in accordance with NDIS Practice Standards, organisational policies and relevant legislation. Support participants to access education, recreation, social, employment and community activities. Participate in team meetings, case reviews, training and continuous improvement activities. To be successful in this role you will need: Relevant qualification in disability, community services, individual support, health or a related field. Previous experience working with people with disability, preferably in an NDIS environment. Strong understanding of person-centred practice, participant rights and duty of care. Ability to develop, implement and monitor individualised support strategies. Good written communication skills and ability to maintain accurate records. Ability to work with participants with complex behaviours or high support needs. Strong communication and interpersonal skills. Current NDIS Worker Screening Check or willingness to obtain one. Current First Aid and CPR certificates or willingness to obtain them. Driver licence and access to a reliable vehicle may be required.
