Are you passionate about making a meaningful difference in the lives of older Australians? We are looking for a proactive and compassionate Care Manager to join our dedicated team, supporting clients across the Adelaide region.

In this client-facing role, you will be the primary point of contact for new and existing home care clients, guiding them through their aged care journey with empathy and expertise. Youll be responsible for delivering high-quality, personalised services and developing strong relationships with clients and their support networks.

Key Responsibilities Include:

  • Build strong relationships with clients and their families

  • Conduct assessments and create tailored care plans

  • Monitor service delivery and ensure client satisfaction

  • Coordinate with nurses, allied health, and internal teams

  • Support client onboarding and promote HCAs services to the community

What Youll Bring to the Role:

  • Strong communication and relationship-building skills

  • Experience in home care, health, or other customer-focused services

  • Confident engaging with clients and stakeholders

  • Proactive, organised, and target-driven

  • Competent with MS Office and CRM systems

Mandatory Requirements

  • A current National Police Check (ACIC accredited), stating suitability to work in aged care or with vulnerable persons.
