Instead of facing adulthood alone at 18, young people aged 17 to 21 are supported, encouraged and championed as they build their futures. As a Transition Coach, you will be one of the steady, trusted adults walking beside them during one of the most important transitions of their lives.This role is primarily community outreach-based, but may include home visits, in-reach to hospitals, detention centres and other locations. You will offer practical support, genuine connection and a belief in their potential. Every conversation, every visit and every moment of encouragement matters.You will be joining a large, passionate and collaborative team that truly values each other. Pay is in accordance with SCHADS Level 4.# **How you will make an impact*** Providing tailored support services using a culturally safe and strengths-based approach and case coordination that is consistent with the Home Stretch WA Transition Coach Practice Principles* Support young people to build natural networks and maintain connection to culture, country, family and community* Assist young people to secure safe and stable housing and navigate the Home Stretch WA safety net resources* Help young people access education, training, employment and meaningful community participation* Walk alongside young people as they build independence, confidence and life skills* Advocate strongly for young people and ensure their voices shape their plans and goals* Work collaboratively with Child Protection staff, carers, kin, community members and other key stakeholders* Conduct outreach across the Perth metro area using a strengths-based and trauma informed approach* Maintain accurate case notes, records and outcomes data# **What you will bring*** Experience working with young people, ideally within the youth work or leaving care sector* A deep understanding of the challenges faced by young people with care experience* The ability to build trust, motivate and support young people through complex situations* Strong case management skills and confidence working independently in an outreach capacity* Excellent communication, relationship building and advocacy skills* A trauma informed, culturally safe and strengths-based approach* Flexibility, initiative and the ability to manage competing priorities* Relevant qualifications in youth work, community services or a related field.* A current WA Driver’s Licence# **What we offer*** NFP salary packaging benefits up to $15,900 tax free and an additional $2,650 Entertainment Benefit* Flexible working arrangements* Employee Assistance Program with confidential counselling* Financial, retail and lifestyle discounts* Discounted health and fitness programs through Fitness Passport* Up to 8 weeks of leave per year through our purchased leave scheme* Generous parental leave of 12 weeks* Up to 5 days paid leave per year to support a TSA program or activity* A supportive, inclusive culture with passionate and dedicated team members* The opportunity to be part of a program transforming outcomes for young people leaving care# **How to apply**If you are passionate about walking alongside young people and want to be part of a positive, collaborative and purpose driven team, we would love to hear from you.
