Instead of facing adulthood alone at 18, young people aged 17 to 21 are supported, encouraged and championed as they build their futures. As a Transition Coach, you will be one of the steady, trusted adults walking beside them during one of the most important transitions of their lives.This role is primarily community outreach-based, but may include home visits, in-reach to hospitals, detention centres and other locations. You will offer practical support, genuine connection and a belief in their potential. Every conversation, every visit and every moment of encouragement matters.You will be joining a large, passionate and collaborative team that truly values each other.
