We are looking for a dedicated Community Worker to join our growing team and support participants in achieving their goals and enhancing their wellbeing. Key Responsibilities As a Community Worker in Enable Community worker, you will be responsible for: · Provide information, assistance, and referrals to individuals to help them access relevant community, health, social, and disability support services. · Advocate on behalf of participants to ensure equitable access to services, resources, and community participation opportunities. · Identify individual barriers to participation and work collaboratively to support inclusion and independence · Assist in identifying needs, strengths, goals, and challenges of participants. · Participate in developing and implementing personalised support or community engagement plans. · Maintain accurate documentation, case notes, and progress reports in line with organisational and NDIS requirements. · Support the planning and delivery of community development programs, group activities, skill-building sessions, and social-participation initiatives. · Monitor and evaluate participant engagement and outcomes, ensuring supports remain aligned with their goals and aspirations. · Help build community capacity by encouraging participation in local networks, events, and opportunities. · Assist participants with building life skills, confidence, and independence (e.g., communication, social skills, problem-solving, community navigation). · Support individuals to safely access the community for appointments, recreation, volunteering, or other meaningful activities. · Encourage participants to pursue their personal, educational, vocational, and social goals. · Work collaboratively with families, carers, allied health professionals, service providers, and local organisations. · Establish positive community relationships that promote inclusion, engagement, and accessibility. Qualifications & Requirements · Minimum Diploma level Education in relevant field. · Minimum 1 year experience as a Community Worker. · Current CPR certification. · Strong clinical judgement, communication, and organisational skills. · Ability to work collaboratively within a multidisciplinary team.
